1) Because the Comics Code wouldn't allow it for some time, back when the Code had teeth.
2) Superhero comics, for decades, were considered kids fare and such graphic violence would have been unthinkable in a comic aimed at kids.
3) A lot of writers are either wussies or editorially mandated to refrain from doing such things in the story. While in reality something like that would happen, the concerns of the franchise and marketability of the characters were always seen as more importnat.
At least, those are all the reasons I can think of. ;)
